The Medway Valley Walk, as its name indicates, roughly follows the River Medway.
Published guide
The original guidebook for the Medway Valley Walk is out of print, but a new guide to the Medway Valley Walk (and other paths) has been published by Kent Ramblers (September 2018). There is some information about the Medway Valley Walk on Explore Kent website.
The route is shown on OS 1:25 000 maps.

The Medway Valley Walk walked on 20 June 2010 (Tonbridge to Maidstone) and 10 July 2010 (Maidstone to Rochester) as part of the Kent valleys project.
